New Patient Appointments

In order to schedule an appointment in our memory clinic, you will need to complete the New Patient Appointment Form.

You can print it and submit it by mail, email or fax. See details below. Once received, it will be reviewed and you will receive a call to schedule the appointment.

If you cannot print a copy and would like one mailed to you, or if you have any other questions, please contact our New Patient Coordinator Coordinator at 813-844-8270.

Alzheimer's Caregiver Workshop

The USF Health Byrd Institute conducts a day-long caregiver workshop at the Embassy Suites- USF. Our next workshop will be held in November 2026.

This conference has been extremely successful and very well attended. Caregivers will learn practical information to help them care for their loved one.
