Welcome to the USF Health Medical Health Administration (MHA) department. The MHA departments purpose is to identify employees, physicians, medical students, health students, and allied health professionals who could be at risk for preventable communicable diseases. Our goal is to minimize the transmission of communicable diseases to and from employees, students, and patients detailed on our Exposures and Resources webpages.

It is required that all professional degree students, residents, fellows, and clinical faculty /staff provided proof of influenza immunization to the USF Health Medical Health Administration office. If you are declining the annual flu immunization due to religious, allergen, or other reasons you must sign and submit the below declination form. Once signed and completed please send to MHADocuments@usf.edu.
